Unite Kansas and Thomas County Coalition join to strengthen community support services


Unite Kansas and the Thomas County Coalition are proud to announce a local collaboration aimed at enhancing access to essential services for individuals and families in Colby, Thomas County, and throughout northwest Kansas. By working together, these organizations will streamline connections between healthcare, social services, and nonprofit organizations to better serve the community.


Unite Kansas, a coordinated care network, is part of the national nonprofit Unite Us. The organization helps connect individuals to vital local resources including food, housing, transportation, peer support and counseling. Through Unite Us' secure software platform,

nonprofit organizations, healthcare providers, and social service agencies can seamlessly send and receive referrals, ensuring that community members receive timely and appropriate care.


“At Unite Kansas, we are committed to building healthier communities by strengthening

communication between care providers and improving access to essential services,” said

Robert DeLeon, Unite Kansas representative. “This partnership with the Thomas County Coalition will help us reach more people in need and ensure that no one falls through the cracks.”


Through this collaboration, Thomas County Coalition and other local organizations will have access to Unite Kansas' network, allowing them to track service referrals, measure outcomes, and identify gaps in care. This innovative approach fosters a more coordinated, efficient system of support that enhances the well-being of residents across the region.


The service provided by Unite Kansas is free, confidential, and fully funded through the Kansas Department for Children and Families' SPARK Grant. Community members seeking assistance can visit uniteus.com/networks/kansas/get-help/get-help to submit a request for support. A trained Care Coordinator will respond within two business days to guide them through the process and connect them with the appropriate resources.


Healthcare providers, social service organizations, and local nonprofits are encouraged to become network partners with Unite Kansas. By joining, they can ensure their clients receive comprehensive care and support while also gaining access to valuable data on local service needs and outcomes.
